Qualifying Conditions Explained: What Physicians Look For
Every state publishes its own list of conditions, and many states also allow physician discretion for conditions not explicitly named. Knowing what to bring to your visit makes the conversation faster.
Commonly evaluated conditions
These conditions appear across most state programs:

Physician discretion
Several states let a licensed physician certify conditions beyond the published list when the clinical picture supports it. If your condition is not named, it is still worth being evaluated.
Documentation that helps
You are not required to bring records, but they can help. Useful items include prior diagnoses, imaging or lab results, a medication list, and notes from a treating provider.
Approval is always determined by the evaluating physician based on your medical history and the standards of your state's program.
