Preparing Your Documents Before an Online Evaluation
Nearly every delayed application comes down to a missing document. This short list covers what physicians and state registries need.
Everyone needs
- A clear photo of the front and back of your driver's license or state ID
- Your current residential address
- A working email address and phone number
- A brief summary of your medical history
State-specific details
Some states require extra information at intake. Texas asks for the last five digits of your Social Security number and your current weight. Georgia asks for your county and driver's license number. West Virginia and Missouri require a full Social Security number. Pennsylvania asks for your state patient ID if you already have one.
Photo quality matters
Take ID photos on a flat surface in good light with all four corners visible. Blurry or cropped images are the most common reason a physician has to ask for a re-upload.
With these ready, most patients complete the full application in under five minutes.
